Itchy, inflamed, red index fingers?

ive had itchy, inflamed, red index fingers for a few years now. ive seen my dermatologist and he wasnt very helpful only suggesting a cream to try and didnt hepl the irritation. ive tried some creams and ointments myself but nothing seems to make it go away completely. iuts not very pretty to look at either. please hepl, thanks :)
